The opening evening is a get acquainted dinner. During the course of the week, be prepared to tell a bit about your self and talk about the photo images you send. The information you send in advance will assist Ricky in getting to know you before you arrive.

Please complete the information survey and send it along with the requested photos/slides to arrive at least a month in advance of your scheduled retreat. You may send your survey and photo either through the post or via email. When sending photos via email, please send only JPEG files. Send your files to justin@rickytims.com and please put LVQR in the subject. Otherwise, please send the questionnaire and images to me at the following address:

What is your name?

What little known fact about yourself - perhaps funny - perhaps interesting, that you are willing to share. (Were you once a fire eater in a circus? A wartime nurse? )

How long have you been quilting?

Provide a personal quote or favorite phrase that might indicate your personality or philosophy? Can be serious or funny – but REQUIRED!

Are you attending LVQR with a spouse or friend? If so, name(s).

Anything else of interest that would help the group know you better?

Please send the following FIVE photos/jpegs.
Two of yourself - one vintage (you choose the age, the older the photo, the better) and one recent photo of yourself. Candid photos are best. You are welcome to choose a photo of yourself pictured with your family/friends/pets. etc.

Send another photo of your choosing, you'll be describing it to the group so pick a good one - any subject (grandkids, pets, vacation home, sentimental, humorous, etc) Include a very brief description of each photo. (when taken etc.)

Also, please send photos/jpegs of two quilts you have made. These also may be professional or snapshot quality photos. Include a description/story of each quilt. Show us your best - your worst, your first, or your funniest.

Please enjoy this little biographical scavenger hunt. It will be well worth your effort. Any hard copies of photos/slides will be returned to you at the retreat. If you have a "precious" original photo, please send a color copy or electronic scan and keep your original.
